basically because of how small and feminine his body looks, and his longish hair he thought he was a girl with probably short/pixie cut hair 
and his face is a lot more feminine n beautiful, and i assume his voice is higher pitch so those signs all lead to him thinking hes a girl 

also in the very first chapter he even mentions that the guy resembled his little sister who passed so he mustve immediately just seen his sister in her
