HEAR ME OUT (it might be a long one)
i don't think it's a time loop or the novel shuffling characters. the accident with the ceiling from MC's memories has made MULTIPLE PEOPLE isekai!!!! probably those who've read the novel *or* those who're close to our MC (can't figure out how the isekai selection works yet). *but* so far we know that MC, sunbae (ML), MC's little sister and this friend have been isekai'ed
now, what we found out from this chapter is that MC and Friend has been isekai'ed during their high school years, which means that *years* have passed since (and, for everyone). we also know from MC that if you don't record your memories from your past life, you lose them. that's how ML has forgotten his own memories but remembers snippets of them (like MC having a younger sister and drawing him with a uniform).
as for Friend, he's probably recorded the memories from their past life and that's how he knows that "his" mom and MC's "dad" aren't their real parents, and that their lives are "unreal". IF Friend is the same non-novel friend from MC's old memories (with the grandma who passed away), ig it is normal to wonder "why're you still sick?" if MC has had a chronic illness before isekai-ing. IDK LMK WHAT YOU THINK

on another note bcs i couldn't put it anywhere in my first comment, but i think MC's younger sister could also be isekai'ed as his little brother bcs all of them were there in that basement at the time of the accident. i don't think kangwu's mention of his little brother was accidental, or how he "grew taller" and whatnot. IDK i myself am skeptical about this particular theory but it is a possibility i think

chapter 6 me was onto something with them two isekai-ing (allegedly, after the incident with the ceiling) and in light of the last two chapters, i have a theory SO HEAR ME OUT
if both MC and his sunbae are in the novel, then just like MC, ML has memory issues (refer back to the "i thought you had a little sister" comment) and "feels" his novel character's emotions/feelings. in light of that, his "why am i like this" comment probably comes from the fact he "knows" he's not supposed to act so kindly towards MC (or be in love with him for that matter) which breaks the events that're supposed to happen in the second act with MC's kidnapping and ML using him to get to Jimin (the novel MC)
*also also* MC preventing muhyeon's (novel ML's) sabotage with their group presentation put him in Jimin's good graces (instead of turning against him like in canon), which is another novel continuity break. like Jimin said, MC's the only guy who doesn't judge him and doesn't ask questions, which COULD mean Jimin might fall in love with him and THAT'S how second act kicks off, with ML kidnapping MC (jealousy or smth idk) and jimin and muhyeon binding together to take ML down
