I'm going say Alice is his older sister, there were complications with the pregnancy and the child did not live and the doctors said the mom wouldn't be able to have another child cuz her body was forever messed up. And so our cute baby was a miracle that wasn't supposed to be possible. And the grandpa was a scumbag that made life miserable for the mom but she didn't know how to be a good parent. Maybe Alice was alive still when he was sick and Alice had a bad immune system so they couldn't risk seeing him or the minor illness coukd have been passed to the sister and she would have died then. So they secretly visited when there was no chance he would talk and germs spittle wound get in the air.
