@Holy water needed they aren't doing anything sexual and he (yurito) isn't enjoying it at all, yurito is trying to save his son from malnutrition and at this point since he physically cannot get near his fiance due to his unstable state this is simply a last resort for yurito, he isn't enjoying any of it he is just doing what he has to to keep his child alive and as healthy as possible.
