The way the the half pig guy was talking about is that they weren't normal like psychotic and that the other people there or subjects all d*ed there except him and little a. The doctor guy seems to have known that the half pig guy follows him because he broke him down since he is dangerous but only recently has he started to act out on his own. The doctor seems to have his own trauma about things so I'm not too sure other than being obsessed with trying to cure the disease that made them half animals
