Just my own thoughts:
Sooha in present day is recovering his real memories, the ones that aren't part of his self made delusions.
He then flashed back to a time when he was still playing his part in his delusion of being from the Omega factory & Haewon is the person who bought him. But during his delusion he broke from it & had a real memory of who Haewon is.
So Haewon, who was hoping that Sooha could recover without forcing him to break from his delusion, but allow him to accept reality from the point he got him from the hospital, chose to play his part of the delusion by being the type of person Sooha thinks he is - which is an Alpha who only wants an Omega to have s** with.
So present day Sooha has to accept that he has real memories from before he was hospitalized, delusions of being in an Omega factory, and that all of his time with Haewon since he was "bought" has been real, but at the same time not cuz each of them played the role he created in his own mind.
Haewon does care for Sooha, but he knows he hurt him in the past & his own behaviors drove Sooha to suicide & that is how he ended up in the hospital. Since Sooha's fragile mental state will only allow him to see Haewon as an Alpha who bought him for s**, he plays that part which is why he sometimes acts cold or uncaring.Spoiler text.
