The medical treatment and medicines are very low grade, I'm sure most of ebis people died because they consumed such medicines and unhealthy food
But it can also be possible that the medicine which emperor's brother gave was actually poisioned because the late king wanted to kill the sick prince but the the emperor's mother took the medicine instead and died
