@Bunnim That yuji eating sukuna's fingers triggered/activated the curse thats been killing ppl at their homes lately and has possibly endangered his sister. He knows yuji will feel bad if he knows that essentially ppl have been dying because of him but what he doesn't know is that yuji already knows this--It's what sukuna was telling and laughing at him about in those panels right after the ones with nobara and megumi.
