He isn't Male lead. She divorces him after the patients are cured. She asks for divorce and proceeds with it and he realizes that she isn't pranking him. And then he asks her to stay as he ig starts to find her interesting. 
She says the coolest lines in the novel explaining if he had said that before and helped her then she wouldn't have suffered so much. And then they divorce. 

Also there isn't a male lead.

For what I have collected your not going to like it but supposedly the dad is the ml with her "brothers" being second and third ml I also there's another one with black hair

So basically he "used to" mistreat because he knew she wasn't kanna but the other girl. Now that kanna is back he is treating her well again.

Okay so huge stuff, kanna is actually related in any way to the Addis family. 
Her mother is the woman who was her mother in the modern universe. And her father is a ghost? 💀 idk but that's what it says in the novel.  Idk why alexssadro (red head dad) took her in. I haven't gotten than far into the novel. But anyways I don't like the red head dad. He and sylvein can go suck di**. Rafayel for the win. 

Kalen and Orsini are basically bunch a nothing. Mostly for plot and our MC's heram. 
The story is great but if they didn't make kanna a part of the Addis family, I think this story would do great.
