I've read through this one a few times, and the beginning moves too fast and doesn't reveal well, so here's an explanation.
Basically, there's a cult active in the city whose members all have this specific tattoo. Our main character, Kyungjoon, is part of the police force that's trying to track them down, but they're having difficulty in doing so bcse the followers keep disappearing without a trace.
Cain is a demon who's fighting with Satan for control over He**, and Satan has injured him with an angelic weapon, lodging a shard into his chest. The shard causes his mana to drain quickly in the human realm so he has to constantly visit He** to recharge. However, while he's in He**, his injury worsens. So he's having to balance going back and forth for energy and self preservation. And the way he travels between the two is through his door!
And his "door" is Kyungjoon. This is, from what I understand, a person who has the capability to open a portal between He** and the human realm, or it can refer to the portal itself. And, since this is BL, the way of opening it is through org***! Or... whatever happened in chapter 3 which is apparently much less pleasant. Though by the way they talk in the aftermath, it was probably something similar?
Most doors slowly lose their soul and go insane when used, but for whatever reason, Kyungjoon is stronger than average and can even withstand Cain's mana.
Also when he travels through the door it's like he steps into or out of Kyungjoon's chest lol
