So basically the imperial family comes from a long line of inbreds and incest that has unsurprisingly caused birth defects on the descendants. Both the emperor and kishiar were born with waaaaayyyyy more power than their bodies could possibly handle and if they go beyond their limits their bodies start breaking down from the inside out until they literally explode. Kishiar's body was close to exploding when he awakened and it somehow stabilised his body.
