Basically, since the towers are nothing more than a relay system to uphold Eshua's world inside a different one, their entire structure is manipulated by the thoughts and desires of whoever conquers them and decides to stay. The first tower was a dark, dead place like the guy playing in there. Belzar's tower was more a game-like dungeon because of Evan's interference. I wonder how much of this will stick.
As for the people, it's entirely possible they were forced into player roles and when they started to run dry, Eshua forcefully brought others from other worlds to play and maintain the towers for her.
