@Milk tea  Mitsu addresses him as a SHE and Naochan did said that he was a biologically born as a guy and he had a ring, like a wedding ring on his left hand. He also did say that Mitsu was lucky to be born as a girl, as of saying that he wanted to be a girl but was trapped in a man's body
