I'm not gonna lie, the author keeps dropping hints that Yeonwoo might have some sort of disease due to his blackouts, nosebleeds and sudden moments of physical weaknes. If he dies at the end of this series, it'll be so sad. But I think it would be a very good ending.

I think it's from some sort of trauma. Psychological issues can also manifest physically e.g. fainting, loss of muscle tone, etc.
