Also, if I remember the earlier chapters correctly, he knows the doc through his own family. That Rui and his family are on more or less bad terms is well shown in the chapters before the kidnapping, when Rui didn't want to take the call and/or see his mother. Also I think he said something along the lines in the earlier chapters that he dislikes the status and everything of his family.

In the end the main plot is done. Shion and Rui are a real couple, loving each other unconditionally. The "bad guys" are either taken out of the game or not a threat anymore.
