It's hard to explain, it's more about intimacy when he calls her mom's name instead of b** in those moments or something. A mix of regret, anger, love, fear, helplessness, traumas, hope, etc, everything, tTat's what he shoul navigate with, and the best/worst part of this whooole story is he almost did it...! Do you remember, he got to that point with B** when he really belived that the boy tried to poison him (agin!), but still, he decided to trust him to the point he restrain himself from an instant act (what he always do anyway...) and actually went and count those pills. He WANTED to be wrong so desperately, he wanted to keep b**, love b**, he really cared him on his own way, and he almost made it,..! He almost reached the point he doesn't have to be kill or even be violent anymore to feel a little bit better, He almost realized he really can calm down, no need to take out his anger because he found someone who loves him so much without any intend to hurt or betray him, so he don't have to be alarmed all the time, no need to worry when this love turns against him... That's a pretty hard thing to learn for someone who was abused (in any way). That you no need to project or take out your anger on anyone, you can handle it on your own, because it's not growing anymore. Because love is not uncertain and hurtful.

I really can't explain it here any further, it's just not that kind of platform where you can discuss these kind of things, I mean, it's not something you can explain in 2-3- or even 30 comments here, it's way more complex than that, it deserves az least an essay, so... Instead, I suggest you check out real serial killers, especially Ted Bundy and Ed Gein. Sangwoo's personality and brain is kinda works as the mix of these two, with this of love-and-hate relationship with his mother, I guess the author inspired by them to some extent. (Ed by the sick relationhip and Ted by the lovely public personality that made him loved by anyone) Also, the Ed Gein case is probably the best and easily understable example to explain why do you start to kill or even ra** people because of you mom, meanwhile you don't have any incest thought.
